CHP Home Care and Hospice provides home health, hospice, and related services throughout Northwest and West Central Ohio. Founded in 1975, CHP is a nonprofit agency with office locations in Ada, Archbold, Bryan, Celina, Defiance, Delphos, Lima, Paulding, Van Wert, and Wapakoneta, Ohio. The organization also operates three adult day centers (Archbold, Celina, Van Wert) and two inpatient hospice centers (Defiance, Van Wert). More information at www.ComHealthPro.org

With nearly 30 years of experience, make CHP Adult Day Centers in Archbold, Celina, and Van Wert, your first choice when a loved one needs care or supervision. Those who attend the day center are referred to as clients. Some are elderly, others are younger with disabilities.
